• Evidence of U.S. Citizenship (original document and a copy of the document)
• Photo identification and a copy of the identification
• Passport photo (can be taken at time of appointment for $15)
• Completed form DS-11 (NOT SIGNED)
• Payment for U.S. Department of State (check or money order)
• Payment for Hoover Library (cash, check or credit card)
• Current or expired passport
• Any name change documents
• Other documents as applicable
